45th Reunion Data Collection Form

 RULES for completing the Form - PAY ATTENTION

You must use the TAB button to move from field to field, NOT THE ENTER BUTTON. Pressing Enter will submit the form. Therefore, RULE 1: Use the TAB button to go from field to field.

RULE 2: IF (or should I say WHEN) you do make the mistake of pressing Enter rather than TAB (old habits are hard to break!!), you can come back to the form by pressing the Return to Home link after your data is listed; enter your First and Last Name again, TAB to the next field you need to complete and finish out the form. We will have two records for you and can merge them prior to creating the directory

RULE 3:  If you have any changes between the time you send us your initial information and September 5 (the cutoff so Betty has enough time to create the directory), come back to this page, select the option "UPDATE" below (rather than the default "INITIAL") and do the same as number 2 above. First and Last name, then TAB to the field(s) to be changed, make the change and TAB to the end of the form, then press Send It On.
